The Application Process
If you live
in the Houston area, apply as a
volunteer through our Houston Chapter, following the steps outlined
below. If you are
accepted, we will prepare you for your Latin American
assignment. Volunteers from other areas should apply by
contacting the Amigos
International Office.
Step one:
Become familiar with the program, to confirm your interest.
Explore this web site, and attend one of our scheduled orientation briefings in
October with a parent.
Step two:
If Amigos is for you, then after the
orientation briefing schedule an
interview for
yourself and at least one parent or guardian.
Step three:
Begin a new
on-line
application. The SAVE button at the bottom takes you through
successive pages of the application. You may exit and later return
to your application (this link is only to return to your existing
application record) to revise or complete it, logging on with the same
email address you submitted on the first page of the application, and
with the password you created there. Further instructions
are available.
Step four:
Complete the on-line application before your
interview, and print it. At your interview appointment, bring the
application
fee of $75 (check payable to Amigos - Houston Chapter), at least one
parent, and the following
printed and signed documents provided in the orientation packet:
- Consent and
Release form
- Release of
Medical Information
- Standards of
Conduct and Behavior form
- Financial
Assistance Information (if applying for aid. Also bring most
recent tax return of the
applicant if independent, or of the person
claiming the applicant as a dependent)
- Medical
Forms. Confidential
Health Form Part I, and Letter of
Medical Disclosure. Seal in the envelope provided in your orientation
packet, and bring to your interview.
The document
forms listed above are available from the on-line application web
site. Also bring to the interview the following additional
documents:
- A
printed copy of the application that you submitted on line.
- Two
photocopies of your passport signature page (if you already have one)
- Release
Waiver and Covenant Not to Sue (from your
orientation packet, and available here)
- Identification
and Medical Release (from your orientation packet, and available here)
- Affidavit
for Single Parent Custody (fill out only if applies - from your
orientation packet, and available here)
Note that
for applicants under 18, all custodial parents or legal guardians must
sign these forms where indicated.
Applicants will be notified of Amigos' decisions for provisional
acceptance by the week of November 2, 2009 pending a successful outcome
of the Confidential
Health Form Part II.
Step five:
Make your own arrangements for a physical examination. Submit
Confidential Health Form Part II, completed by your physician or
clinician, sealed in
the envelope provided at orientation. This will be due
on or before December 13, 2009.
